You need Dejap's conversion utility to deinterleave it, no other utility does it right.

Well, chances are you have the interleaved (totally fucked up) format. You need the game in deinterleaved (unfucked) format for the SWC DX2. The only thing I found that works on it is Dejap's conversion utility. You have to convert the Japanese ROM using their utility and then apply the conversion patch. After this you may still need to fix the header. Since Dejap's page seems to be down (last known location www.dejap.com), I'll e-mail you the fix.
