I spent much of the day looking at this. I think your main problem is you need a good header on the ROM. The Game Doctor is also very strange and seems to be picky about the file name you assign to your ROM. I still don't understand much about the Game Doctor header (if anybody has any info feel free to pass it along) but give this a try Super Metroid - Game Doctor SF7.
