I have played all the way through a PAL Terranigma cart on a US SNES (modified with 50/60Hz switch). There were no glitches or problems as far as I could tell.

A couple of minor points.
One way of running in the game is to tap twice on the D-pad. In 60Hz mode this is slightly trickier to do because the timing needs to be quicker.

At one point in the game, there is (from memory) some kind of food-eating contest, where you have to press alternate buttons very quickly to win. This is very hard in 60Hz mode.

The PAL version timings *might* have been adjusted for 50Hz play. But I haven't played the NTSC (Japanese) version of Terranigma, so cannot say for sure. Either way, it is not a major problem.
